MHA LABS 21ST CENTURY SKILLS ASSESSMENT (HAQ) 

Thank you for being a dedicated youth development advocate! This developmental 21st century skills assessment was developed to support two critical youth development goals:

• To provide objective feedback to your youth participant about his/her specific skill strengths and areas for development to increase their success in school, home, college, career and life.

• To provide your youth participant with documented information that supports skill-based goal planning. Youth may use the completed Scorecard in their personal development portfolio. This Scorecard may help them communicate their strengths and growth areas to their next instructor or mentor.

DIRECTIONS

This tool has three targeted sections that provide youth with a detailed, holistic understanding of their skills development profile.

• Section 1 = Context questions provide background on the development experience
• Section 2  = Skill Ratings pinpoint the specific skills driving your youth’s program performance
• Section 3  = Satisfaction survey questions rate overall performance in the program
